1. Continued Care Financial Overview By signing this agreement, you (“Client”) are entering into a monthly membership with GT3 Health. Monthly membership dues Billing occurs on the 1st of every month.
2. Payment Terms The membership dues will be automatically billed on the 1st day of each month to the payment method on file. It is the Client’s responsibility to ensure payment information remains current. Failed payments may result in a suspension of services until payment is resolved.
3. Cancellation Policy A 30-day written notice is required by the Client to cancel this membership. Notices must be submitted by email to: hello@gt3health.com. Cancellations will take effect 30 days from the date the notice is received. No partial refunds will be issued for mid-month cancellations.
4. Agreement Duration & Renewal This agreement is month-to-month and will continue until terminated with proper notice. Fees and terms may be updated with 30 days’ notice to the Client.
5. Acknowledgment By signing below, the Client acknowledges and agrees to the terms above, including the billing and cancellation policy.
